How Our Nursing Team Can Support You

Clinical Assessments & Care Planning

Our nurses conduct thorough clinical assessments to understand your health needs and design individualised nursing care plans. These plans are developed in consultation with you and your support network to reflect your goals, preferences, and NDIS planning outcomes.

Medication Management

We assist with safe medication administration, monitoring and documentation, ensuring medications are taken correctly and any changes in health are reported promptly.

Chronic Condition Monitoring

For participants with conditions such as diabetes, respiratory concerns or cardiovascular needs, our nurses monitor symptoms, provide education and work closely with your GP or specialists.

Wound & Skin Care

Specialised nursing support includes wound assessment, dressing changes and ongoing monitoring to promote healing and prevent complications.

Catheter Care & Management

JESK Care provides professional and dignified urinary catheter care for participants who require ongoing clinical support. Our nurses deliver catheter care in line with best practice guidelines, with a strong focus on comfort, privacy, and infection prevention.
Our catheter care services include:
- Indwelling (IDC) and suprapubic catheter care
- Catheter insertion, removal, and replacement (as clinically required)
- Monitoring for infection, blockage, or complications
- Catheter hygiene and routine maintenance
- Education for participants and carers on catheter management
- Clear documentation and communication with GPs and healthcare teams
We understand catheter care can be sensitive, and our nurses provide support in a respectful, person-centred manner to promote confidence, safety, and wellbeing.

Health Education & Support

We provide personalised education to participants and their carers to increase understanding of health conditions, medication routines, self-management strategies and when to seek urgent care.

High-Level Clinical Support

Where more complex care is required, such as enteral feeding or ongoing clinical monitoring, our nursing team delivers safe, professional support that complements your broader NDIS support plan.
